Background

I have been developing Quick References since February 2011 to support Primary Care Teams in Tower Hamlets, London to co-produce care with people who are in their last years of life. In my role as GP Clinical Lead for care in the last years of life I developed the statement here:

I have worked closely with Chiu Mayes-Loh from Sparrow Design to develop this work to improve people’s lives through communication design. Sparrow Design has roots in social enterprise and in the regeneration of East London’s local economy.

Quick References developed in 2010, 2011, 2014 (detailed below).

The cumulative research done to develop these references led to the conceptualisation of the Handy Approach© as part of a Quality Improvement Project whilst working at the East London NHS Foundation Trust  delivering integrated person centred care from 2014-2016.

Our Quality Improvement work found that the Handy Approach© was safe, easy to learn and easy to use, with 100% team uptake.
